Geriatric nursing and senior care management have been the core of my practice for 35+ years. As a Concierge Senior (Geriatric) Care Manager, I assist clients and their families explore different options to either ‘age in place at home’ or find the right setting where they can receive the best care, (sometimes an assisted living or skilled nursing facility). A true patient advocate.
Sometimes clients may do well with a short rehabilitation stay at a subacute facility, allowing them to regain their strength and endurance, and return home and live safely and independently. Home visits are often necessary to evaluate for safety and the need for equipment to make your loved ones more comfortable and safe. As a Geriatric Care Manager, I further assist clients and families explore options for paying for services through Supplemental Medicare insurance, Medicaid, or Long Term Care Insurance. Often it is necessary to work together as a team with an Elder Care Attorney, who will provide the legal aspect of planning the care for a loved one.
PRI (Patient Review Instrument) and Screen in New York State is for Skilled Nursing Home Placement. The PRI is a clinical tool that helps to determine how much assistance your loved one will need in the skilled nursing facility.
GCM is planning and coordinating care of the elderly with physical and/or mental impairments to meet their long term care needs, improve their quality of life, and maintain their independence for as long as possible.
A full patient assessment is completed, wounds are assessed for etiology, prevention, risk factors, treatment options, nutrition, durable medical equipment needs, and any additional care needs.
A complete home care assessment is an essential step when coordinating care for an elderly loved one so that we can create a care plan that addresses those needs, and helps to keep them as safe as possible.
“Sharon was great to work with. She is extremely knowledgeable, caring and compassionate. It is very tough to have to make difficult decisions about loved ones, Sharon was very patient, and really helped guide us. She worked very hard for us, to make sure my father would be taken care of by some of the best caregivers out there. I highly recommend working with Sharon.”
“Sharon has been a lifesaver during an extremely challenging and difficult time. I have been thrilled with her service and I’m extremely happy that I found her. Her knowledge, experience and contacts in this industry are extraordinary.”
“Sharon tirelessly advocated on my father’s behalf to request a reassessment by Medicaid, arrange additional evaluations by doctors and physical therapists, and provide all the required medical documentation. After several weeks of back and forth, the agency approved 24-hour care last month. Without Sharon’s knowledge of geriatric care and skill in negotiating with the agencies, we would not have been able to get the help my dad urgently needed. In addition to her expertise, Sharon’s kindness, warmth, and quick responses to our questions helped to alleviate our stress during this difficult time.”
