Geriatric nursing and senior care management have been the core of my practice for 35+ years. As a Senior (Geriatric) Care Manager, I assist clients and their families explore different options to either ‘age in place at home’ or find the right setting where they can receive the best care, (sometimes an assisted living or skilled nursing facility).
Sometimes clients may do well with a short rehabilitation stay at a subacute facility, allowing them to regain their strength and endurance, and return home and live safely and independently. Home visits are often necessary to evaluate for safety and the need for equipment to make your loved ones more comfortable and safe. As a Geriatric Care Manager, I further assist clients and families explore options for paying for services through supplemental Medicare insurance, Medicaid, or Long Term Care Insurance. Often it is necessary to work together as a team with an Elder Care Attorney, who will provide the legal aspect of planning the care for a loved one.
An attorney can assist families to obtain legal documents for power of attorney (POA), health care proxies, living wills, asset protection, trusts, and pooled trusts. Pooled Income Trusts can be created to preserve income when one applies for Medicaid for Home Care (also known as Community Medicaid). Furthermore, I will assist clients and families to understand the eligibility requirements for Medicaid for Home Care vs. Nursing Home Medicaid. Through my many years of experience as a home care nurse, I have unlimited resources to assist with almost any need for a client, (senior programs, private hire aides, certified home health agencies, home visiting MD’s/ NP’s/ and PA’s).

PRI (Patient Review Instrument) and Screens in New York State are for Skilled Nursing Home Placement. The PRI/Screen is a clinical tool that helps to determine how much assistance your loved one will need in the skilled nursing facility.
GCM is planning and coordinating care of the elderly with physical and/or mental impairments to meet their long term care needs, improve their quality of life, and maintain their independence for as long as possible.
A full patient assessment is completed, wounds are assessed for etiology, prevention, risk factors, staging, nutrition, durable medical equipment needs, and any additional care needs.
A complete home care assessment is an essential step when coordinating care for an elderly loved one so that we can create a care plan that addresses those needs, and helps to keep them as safe as possible.
