PRI (Patient Review Instrument) and Screen in New York State is for Skilled Nursing Home Placement. The PRI is a clinical tool that helps to determine how much assistance your loved one will need in the skilled nursing facility.
A full patient assessment is completed, wounds are assessed for etiology, prevention, risk factors, staging, nutrition, durable medical equipment needs, and any additional care needs.
A complete home care assessment is an essential step when coordinating care for an elderly loved one so that we can create a care plan that addresses those needs, and helps to keep them as safe as possible.
